Dear Patient,
This letter is to inform you of my decision to close Sulkowski Family Medicine, PLLC, on June 20, 2025. I am closing the practice due to financial constraints and unstable staffing. I am deeply saddened to come to this decision.
It is important that you continue to receive medical care. You will need to find a new health care provider. You should contact your health plan to get a list of providers in your network. I will not be able to provide medication refills after June 20, 2025. If you anticipate needing refills, plan early and contact our office. If you haven’t been seen in the past six months, you will need to make an appointment.
I have contracted with a medical records custodian, Cariend, to manage requests for your medical records for the next 10 years. Cariend is not a medical provider and does not handle billing, medications, or referrals. Cariend will be able to start accepting requests for my patients’ records after July 1, 2025. You should be prepared to complete a Records Request Authorization Form and provide a copy of a government-issued photo ID, as required by HIPPA. The fastest way to request a copy of your medical records is to visit Cariend’s website https://www.cariend.com/request-records/ and follow the instructions.
If you do not have access to a computer or internet, Cariend can be reached by phone at 404-991-7080. If you have questions about the process, Cariend can also be contacted via email at Request@Cariend.com. You should not send sensitive personal information or request documents through email. Please note the HIPPA laws do not allow medical records to be released based on a phone or email request. Beginning July 7, 2025, I will be joining the medical team at CenterWell Senior Primary Care in Murfreesboro, TN. This practice is limited to senior patients on Original Medicare or several of the Medicare Advantage plans. I welcome any of you who are on one of the eligible plans to join my patient panel at CenterWell.
