Christmas and New Years Holiday hours

The office will be closed on Monday, December 24, 2018 and Tuesday, December 25, 2018 for the Christmas Holiday.
The office will be closed on Monday, December 31, 2018 and Tuesday, January 1, 2019 for the New Year Holiday.

Extended hours
We would like to remind our patients that we now are open Monday through Friday, 8:00 am to
5:00 pm for appointments. We will continue to be in the office through lunch, but our phones will be rolled to voicemail from 12:00 pm to 1:00 pm. Our staff checks their voicemail regularly throughout the day to take care of your requests and questions.
Dr. Sulkowski hopes the extended hours will be more accommodating to patients and help those needing Tuesday and Thursday afternoon appointments. Give the office a call and Kelly, our receptionist, will get your appointment scheduled as soon as available. Our goal is to get our patients in for an appointment same day, if that is what you need.

We are now Sulkowski Family Medicine
We would like to welcome our patients to the new Sulkowski Family Medicine. Dr. Sulkowski is no longer affiliated with Family Practice Partners. The same staff will be helping you with your health care needs. We are still in the same location, 1810 Ward Drive, Ste 101, Murfreesboro. Our phone number remains the same, 615-900-1381.
