Dr. Sulkowski grew up in State College, Pennsylvania, and earned his Bachelor of Science from Pennsylvania State University. He earned a Master of Education in Athletic Training from the University of Virginia. He worked as an athletic trainer in collegiate athletics, professional sports, and Olympic sports festivals for five years. Dr. Sulkowski earned his medical degree from the Medical College of Virginia-Virginia Commonwealth University. He completed his residency training at Chesterfield Family Practice-Virginia Commonwealth University in Richmond, Virginia. He has been certified by the American Board of Family Medicine since 1990.
Dr. Sulkowski worked in Family Medicine practices in Knoxville, Tennessee, and Savannah, Georgia, before relocating to Murfreesboro, Tennessee, in 1998. He has been married to his wife, Julie, for almost 30 years, and they have four children.
In his free time, Dr. Sulkowski enjoys spending time with his family and participating in sporting activities, especially golf. He is a member of the Knights of Columbus and St. Rose Church.
Dr. Sulkowski believes his most important attribute as a physician is his ability to listen to his patients. He takes the time to take care of his patients' concerns. He is committed to his practice motto of "family care from the heart."
